The initiative, which launched in July 2025, is focused on enhancing the care and management of patients with uncontrolled hypertension—a condition affecting over 30% of adults in Ohio, leading to thousands of preventable hospitalizations annually, offering continuous support to improve outcomes and reduce hospital visits.

Once the rollout is complete to the UH Primary Care Institute, RPM will span nearly 100 practice sites and support over 400 providers.
