The initiative, which launched in July 2025, is focused on enhancing the care and management of patients with uncontrolled hypertension—a condition affecting over 30% of adults in Ohio, leading to thousands of preventable hospitalizations annually, offering continuous support to improve outcomes and reduce hospital visits.
