HealthSnap Recognized as Quarterfinalist for the Digital Health Hub Foundation: Digital Health Awards

MIAMI, September 3, 2024— HealthSnap, a comprehensive virtual care management company, is thrilled to announce its selection as a quarterfinalist for the prestigious 2024 Digital Health Hub Foundation: Digital Health Awards. The Company was chosen as a quarterfinalist for the Best in Class track – Home Health Diagnostic category.

This recognition highlights the company’s dedication to shifting the paradigm in the way chronic conditions are managed – from a model that is reactive, episodic, and delivered in the clinic to one that is proactive, ongoing, and delivered in the home. As a result, patients on HealthSnap’s programs are experiencing more proactive and personalized care, leading to improvements in outcomes and accelerated adoption by healthcare providers across the country. The Digital Health Awards celebrate organizations that demonstrate excellence in leveraging digital advancements to enhance healthcare services and patient outcomes. As a quarterfinalist, HealthSnap stands out for its commitment to improving patient and provider experience.

This recognition highlights HealthSnap for its significant contributions to helping healthcare organizations improve patient outcomes, reduce utilization, and diversify revenue streams through chronic disease-agnostic Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Principal Care Management (PCM) programs. The Digital Health Hub Foundation Awards honor organizations making strides in improving healthcare speed, efficiency and equity with their digital innovations. Being named a quarterfinalist reflects HealthSnap’s commitment to driving meaningful change in the healthcare landscape.

“We are honored to be selected as a quarterfinalist in this year’s Best in Class track for the second year in a row. In the last year, our team has announced multiple new health system partnerships, and recently released our most robust clinical outcomes report to date,” said HealthSnap CEO and Co-Founder Samson Magid. “We are only just getting started when it comes to the long-term impact we hope to make in the lives of patients and providers using HealthSnap’s virtual care management platform and programs, and this recognition by the Digital Health Hub Foundation is a testament to the ongoing dedication of our team and partners.”

Out of a substantial pool of over 1,500 submissions, only the most promising 16 quarterfinalists per category, per track have advanced to this stage.

“This year has set a new benchmark for excellence, with a remarkable influx of global submissions showcasing groundbreaking innovations in digital health. The level of ingenuity and commitment we’ve seen is truly inspiring.
