Premier Cardiovascular Care of Dallas is Transforming Hypertension Care with Remote Patient Monitoring

This practice turned to HealthSnap’s Remote Patient Monitoring program to bring clarity, consistency, and proactive engagement to hypertension management.

“Managing blood pressure on patients had been challenging because of inconsistency in blood pressure at home and in the office. HealthSnap helped with this matter,” said Dr. Amit B. Guttigoli, MD, FACC.

The Practice’s Starting Point: Inconsistent Blood Pressure Readings

Premier Cardiovascular Care of Dallas faced a familiar challenge: managing hypertension effectively when patient blood pressures looked very different in the office compared to at home. Without a clear, consistent picture of patient data between visits, it was difficult to assess true control, adjust medications, or intervene at the right time.

“Managing blood pressure on patients had been challenging because of inconsistency in blood pressures at home and in the office. HealthSnap helped with this matter,” said Dr. Amit B. Guttigoli, MD, FACC.

The practice sought a solution that could bring clarity, consistency, and proactive engagement to hypertension management.

The practice turned to HealthSnap’s Remote Patient Monitoring (RPM) program to bring clarity, consistency, and proactive engagement to hypertension management.

The Challenge: Limited Visibility Between Visits

Like many cardiovascular practices, Premier needed a way to:

  • Track patient progress in real time rather than waiting months between appointments.
  • Identify when blood pressure was trending out of control before it led to complications.
  • Increase patient accountability and adherence to treatment plans.

These challenges highlighted the need for a new approach that extended care beyond the office visit.

HealthSnap’s Solution: Data + Human Connection

To close these gaps, Premier implemented HealthSnap’s RPM program for hypertension. The program provides:

  • Daily monitoring of home blood pressure readings, ensuring early outreach when values fall outside target ranges.
  • Dedicated licensed nurses and care navigators who provide education, encouragement, and accountability between visits.
  • Integrated reports and trend analysis that equip providers to make timely, informed clinical decisions.

This combination of technology and human touch created a consistent feedback loop, improving accuracy, engagement, and adherence.

Clinical Outcomes That Matter

Premier Cardiovascular Care of Dallas didn’t just gain clearer visibility into patient health, they achieved measurable improvements that made a real difference for both patients and providers. In just six months, the program delivered impactful results:

Within the first six months, the proportion of patients with uncontrolled hypertension fell by one-third, dropping from 55.3% to 37.1%.

One in three patients moved out of the uncontrolled range in just six months — a rapid improvement in hypertension control.

Among 132 patients who began in the hypertension range (SBP ≥130 mmHg or DBP ≥80 mmHg), average blood pressure dropped 12.3/-7.7 mmHg, with 80% showing improvement. Patients also demonstrated high engagement, transmitting readings on two-thirds of days — more than 20 readings each month on average.

132 patients lowered blood pressure by an average of 12.3/-7.7 mmHg, with strong engagement driving 20+ data submissions per month.

Medication compliance also improved significantly, as patients became more involved and consistent in following their treatment plans.

“Our patients’ blood pressures are under better control, and their medication compliance has improved significantly. We’re seeing real changes in both outcomes and patient engagement,” said Dr. Guttigoli.

The Experience: Personalized, Ongoing Support

Beyond the data, what stood out was the personal connection patients developed with their care team.

“The human touch that HealthSnap brings is vital. The navigators and nurses have a rapport with our patients, and patients are very happy with the service,” said Dr. Guttigoli.

HealthSnap’s combination of proactive monitoring, dedicated care navigation, and easy-to-use technology ensured that care extended far beyond the clinic walls, helping patients feel supported every step of the way.
