Heart & Vascular Associates
“They’ve been an excellent partner,” said Dr. Piotrowski. “My only regret is not doing it sooner.”
At HealthSnap, we’re committed to empowering healthcare providers to deliver exceptional care through our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) platform. Our partnership with Heart & Vascular Associates, a leading cardiology practice, showcases how our solutions drive meaningful clinical outcomes and enhance patient satisfaction. Here’s how Heart & Vascular Associates improved patient satisfaction and clinical outcomes with HealthSnap, as shared by Dr. Bryan Piotrowski.
Overcoming the Challenges of Chronic Care Management
Heart & Vascular Associates, a cardiology practice serving a diverse patient population (average age 74.6, ranging from 32 to 98 years, 56.5% female), recognized the potential of RPM and CCM to reduce hospital admissions and improve patient outcomes. However, launching these programs in-house was daunting. Dr. Piotrowski explained, “We’ve seen the data that it’s been helpful in reducing hospital admissions and ER visits, but the more we learned, the more daunting it became to start a program on our own.”
After evaluating multiple vendors, Heart & Vascular Associates chose HealthSnap for our comprehensive virtual care management platform and long-term partnership approach. “This is not a short-term outreach,” Dr. Piotrowski noted. “We wanted something for our patients for the long run.”
Impactful Clinical Results with HealthSnap
Since partnering with HealthSnap, Heart & Vascular Associates has enrolled 597 patients in their RPM program, with an average duration of 287.8 days, and 293 patients engaged for over a year. Patients transmit readings on 68.7% of days—approximately 21 days a month—demonstrating strong engagement. The results speak for themselves:
- Significant Blood Pressure Reductions: Heart & Vascular Associates patients with baseline blood pressure levels in a hypertensive range averaged a reduction in systolic blood pressure of 7.5 mmHg.
- Fewer Hospitalizations and ER Visits: Proactive monitoring resulted in a decrease in hospital admissions and emergency department visits.
- Enhanced Patient Satisfaction: Improved communication between patients and the practice has fostered greater trust and engagement.

Among the 322 patients whose baseline blood pressure fell within a hypertensive range, highly significant reductions were observed in both systolic and diastolic values, averaging -7.5 mmHg and -5.6 mmHg, respectively. Nearly half of these patients began their programs in the Stage 2 Hypertension category (defined as ≥140/90 mmHg) and experienced average reductions of -15.3/-9.5 mmHg over an average program duration of 305 days.

The proportion of patients meeting controlled hypertension criteria increased steadily over time, rising from 43.9% in their first month to 60% among those enrolled for 14 months.
Dr. Piotrowski highlighted, “We’ve seen systolic blood pressure reductions of 10 mmHg, which is comparable to the effect of adding a new hypertension medication. To achieve that through a non-pharmacologic intervention like RPM is incredibly meaningful. It’s made a real difference for our patients.”
Why HealthSnap Exceeds Expectations
HealthSnap’s platform and support have gone beyond Heart & Vascular Associates’ expectations. Key highlights include:
- Seamless Support: Monthly meetings allow Heart & Vascular Associates to review detailed analytics, troubleshoot issues, and optimize care. Dr. Piotrowski praised, “The customer support, the care navigators, the ease of use for our patients in the portal—all that makes a tremendous difference.”
- Patient-Centric Design: Our cellular-enabled devices and intuitive portal empower patients, even those less tech-savvy, to engage consistently with their care.
- Data-Driven Insights: Regular outcomes presentations show continued improvement, with even greater benefits expected as more patients surpass the one-year mark.
“They’ve been an excellent partner,” said Dr. Piotrowski. “My only regret is not doing it sooner.”
