Summary

Care Station Medical Group is a joint venture practice of Robert Wood Johnson Barnabas Health, and is proud to participate in Medicare’s Primary Care First Program. Care Station is held to the highest standards as a medical practice to provide primary care, urgent care, enhanced patient access, and innovative ways to promote health and wellness to their patient population. Care Station Medical Group is an Open-Access Primary Care Practice that takes a proactive approach to patient care and escorts patients through the complex maze of healthcare. Care Station Medical Group’s Primary Care practice has six locations across New Jersey.

Challenge

The team at Care Station Medical Group shared that ​​before implementing Remote Patient Monitoring (RPM), they followed traditional protocols in order to manage patients’ ongoing chronic conditions, for example, they would see patients at the clinic and adjust medications based on in-office testing. 

Care Station decided to implement RPM because the team recognized its potential to improve patient care by enabling continuous monitoring of vital health metrics outside of traditional healthcare settings. Michael Dardia noted, “This proactive approach improves blood pressure control and allows for early detection of health issues and timely intervention, ultimately leading to better outcomes for our patients.”

Program Highlights

Jigna Pokhan shared, “What particularly stood out to us about HealthSnap was their team’s flexibility to allow us to implement a successful enrollment program, as well as the accessibility of support throughout our collaboration.” She noted that Care Station’s dedicated HealthSnap Customer Success Manager, Celie, has been extremely responsive. This was highlighted by the support she provided to the clinician chiefly responsible for onboarding enrolled patients in troubleshooting at the point of service so that patients were enrolled into the program easily and efficiently.

In terms of patient outcomes, Jigna noted a marked improvement in controlling both systolic and diastolic pressures (see detailed clinical outcomes data below).
